First off, they have to win out.

Then, AZ has to lose at least once after playing STL. DET and Dallas have to go winless the rest of the way.

I think SF has to lose out or win one game, vs AZ week 17. If that happens, Rams are the 5th seed. If AZ wins week 17, and everything else goes right, Rams the 6th.
